This persistence is observed even though secondhand smoke poses significant health risks, including respiratory illness, cardiovascular disease, and lung cancer, and ventilation systems are largely ineffective at fully removing toxic tobacco pollutants.

Policies regarding airport smoking are primarily determined by national laws rather than universal aviation standards, leading to a fragmented global landscape. For example, airports in countries with strict indoor air quality laws, like the United States and Australia, generally enforce total bans on indoor smoking. However, many hubs in Asia and the Middle East continue to provide enclosed, ventilated smoking rooms, often seen as a compromise between passenger comfort and public health mandates. The International Civil Aviation Organization (ICAO) offers guidance but does not mandate a global ban on indoor smoking.

In the US, there is no federal ban on smoking inside airports; indoor smoking policies are driven by state and city clean-indoor-air laws and individual airport authority policies. As of April 2025, 32 of the 35 busiest US airports were 100% smoke-free indoors, but Las Vegas (LAS) and Nashville (BNA) airports still maintain indoor lounges. Hawaii has stricter rules, banning smoking even curbside. The persistence of these indoor lounges in some airports can correlate with local tobacco control legislation that allows smoking in hospitality settings, or be influenced by the economic benefit of converting these spaces into premium retail or dining options, which significantly increases terminal revenue.

The trend in the UK, however, shows a move towards zero-tolerance enforcement past security, with multiple major airports like those in London, Manchester, Birmingham, and Edinburgh systematically dismantling airside smoking facilities. This shift is driven by prioritizing green energy standards and premium retail space, with searches for "airport smoking areas" increasing by 5,000 percent in the past month as travelers face unexpected changes. Employees in smoke-free airports report fewer upper respiratory symptoms and lower levels of cotinine, a biomarker for nicotine exposure, demonstrating measurable health benefits from eliminating indoor smoking areas.
